(Effective September 1, 2010)
In addition to your personal information (name, address, phone number), the OCF-6 requires a list of goods and services you wish to claim as expenses resulting from your accident. This list will include a description of each good and/or service, including the names of service providers. In addition, you need to enclose the many bills and receipts from those goods and services or explain why you cannot provide these.
- assistive devices required because of your accident (e.g., walkers, prosthetics, crutches, wheelchairs).
- caregiver and/or attendant care benefits (optional).
- clothing or personal belongings lost, damaged, or destroyed in the accident.
- clothing or personal belongings lost, damaged, or destroyed in the accident.
- housekeeping and home maintenance (optional).
- lost educational expenses (e.g., non-refundable tuition, in case you cannot attend classes due to your injuries).
- transportation costs incurred because of your injuries.
- travel expenses for visitors whose help you need to recover.
